We are pleased to present the 2019-2020 updated facility and provider group influenza tool kit. In 2019, we noticed a variance in influenza monitoring, treatment and chemoprophylaxis throughout facilities and health care systems. We acknowledged patients would be better treated if there was a standardized process as it would shorten the time of drug to patient during confirmed cases and outbreaks in facilities. We worked with a DNP student from the University of Minnesota. Current facility policies and health care system orders were reviewed and reconciled prior to staringt the project. Literature was reviewed and drafts were vetted by the providers of Metro Alliance multiple times. We encourage this protocol and order set to be used by the Medical Director and DON to plan for a potential influenza outbreak and using the Chemoprophylaxis order set. Having a discussion and making a decision about which renal function calculation to be used ahead of time will help facilitate fast initiation of chemoprophylaxis treatment. Please note, that if the Medical Director and DON decide to use CrCl, it would be imperative to understand who would do the calculations prior to the orders being signed. 2019-2020 brought an updated protocol that includes:
The option of using GFR or CrCL per medical discretion
Adding INR draw when started on pharmacological intervention
Improved/cleaner order options
An additional page of Dosage Adjustments Order based on new renal functions
Improved editing and formatting to allow the first and second page to be printed back to back to allow a “1 sheet” order set.
